You wish to make an appointment with us for a COVID-19 Screening Test? You can download here the complete procedure.

 

Thank you to book an appointment through our Appointment Request Form and provide us with:

  • Copy of your Passport 
  • Download and complete the  Laboratory request form 
  • Phone Number is mandatory for communication of the result 

Your appointment will be confirmed once we have received and checked all these documents.

The day of your appointment:

  • Present yourself to the front desk receptionist.
  • Follow the instructions given by the receptionist.
