Epidemiology and Public Health Unit

The Epidemiology and Public Health Unit of the Pasteur Institute of Cambodia conducts operational epidemiological research studies on major public health challenges in Cambodia. Specifically, the work carried out aims to improve the understanding of the transmission of infectious and emerging diseases and to improve the prevention, diagnosis and treatment of priority pathologies in the most vulnerable populations.

The Epidemiology and Public Health Unit of the Pasteur Institute of Cambodia conducts operational epidemiological research studies on major public health challenges in Cambodia. Specifically, the work carried out aims to improve the understanding of the transmission of infectious and emerging diseases and to improve the prevention, diagnosis and treatment of priority pathologies in the most vulnerable populations.

Main Missions and Team Presentation

Our mission is to implement epidemiological and clinical research studies, based on a cross-sectional and multidisciplinary approach, and to make recommendations for interventions and health policies based on scientific knowledge and evidence. The purpose of the actions carried out within the unit is thus to understand and improve the health and well-being of populations and to reduce health inequalities.

Our research activities call upon different disciplines including epidemiology applied to infectious diseases and environmental health, clinical research, biostatistics, mathematical modeling, health geography as well as human and social sciences applied to health. It is most often based on the collection and analysis of data relating to population samples, more or less large depending on the objectives and designs of the various research projects.

The main research themes studied in recent years target specific public health problems in Cambodia and more broadly in Southeast Asia including rabies, arboviruses, HIV, Tuberculosis, Malaria, Nipah virus, Antibiotic resistance, Leptospirosis, Influenzae viruses, Sars-CoV-2, etc…

In 2023, the unit, composed of 52 people, is organized around 5 specific and complementary groups including: 

  • The Community Epidemiology Group, led by Dr. Sowath Ly, who has extensive experience in community epidemiology research projects on rabies, dengue, avian influenza and investigations of epidemics and health alert situations; 
  • The Clinical Research Group, led by Dr. Nathalie de Rekeneire, which pilots clinical trials and contributes to guide international prevention, diagnosis and treatment strategies for vulnerable populations with HIV, tuberculosis, hepatitis and some vaccine preventable and emerging infectious diseases. 
  • The One Health group, led by Dr. Hélène Guis, which arises from the collaboration between CIRAD and IPC to tackle the human, animal and environmental components of disease transmission and prevention in a constructive and coordinated One Health approach. Its focus is on zoonotic and vector-borne diseases. 
  • The Methodology and Data Analysis group, coordinated by Dr. Claude Flamand, which aims to develop and implement methodological tools and approaches adapted to the studies carried out including study designs, data management and analysis resulting from the research work carried out; 

Main Field of Research

Improving Prevention, Diagnosis, and Treatment of Infectious Diseases in Vulnerable Populations

 

Despite significant progress, Cambodia continues to face serious challenges in detecting and preventing Tuberculosis (TB) cases, particularly among people living with HIV. Many challenges also remain in reducing the prevalence of HIV, TB, and exposure to viral hepatitis B or C in vulnerable populations. The lack of access to efficient, affordable, and easy-to-use diagnostic tools and late screening in certain exposed populations are still important challenges for health authorities to reduce the burden of morbidity and mortality. The clinical research group of the Epidemiology and Public Health Unit conducts important research activities aimed at improving access to diagnosis and systematic and rapid detection of HIV or TB in exposed populations. This work contributes to important multicenter randomized trials evaluating the impact and cost-effectiveness of new therapeutic approaches, the effectiveness of new molecules, or the performance of new diagnostic tools, providing “authorities with valuable information to enhance the management of the studied diseases”.

Assessing Immunity after Rabies Post-Exposition Prophylaxis

 

International health authorities support a major effort to eliminate canine rabies in humans by 2030. While this goal could be achieved in the long term by mass vaccination of dogs, improving access to adequate and timely post-exposure prophylaxis could save lives in the short term. The Epidemiology and Public Health Unit conducts studies on the immune response induced by post-exposure prophylaxis to provide authorities with useful knowledge to optimize recommended vaccine regimens and improve compliance in exposed populations. This work improves the knowledge on the characteristics of protection, humoral and cell-mediated response related to vaccine regimens and contributes to the adaptation of the WHO recommendations for rabies post-exposure prophylaxis.

Investigating the Circulation of Zoonotic Pathogens in a One Health Perspective

 

One Health (OH) is an integrated multisectorial and multidisciplinary approach that recognizes the close links and interdependence of human, domestic and wild animal, plant, and environmental health. OH approaches are key to better understand, prevent, and control zoonotic pathogens. In Cambodia, 80% of the population lives in rural areas, and most households own animals (poultry, swine, cattle, dogs), with common contacts with wildlife (consumption, tourism…). The Epidemiology and Public Health Unit develops cross-sectional or longitudinal surveys in which the health of humans is monitored alongside the health of domestic or wild animals to identify reservoirs, evaluate prevalence and incidence of zoonotic pathogens in the different hosts in space and time. The aims of these approaches are to better understand where, when, how transmission takes place (frequency, duration, location, circumstances, types of contacts), the role of animals (reservoirs or sentinels) in pathogen circulation, which prevention or control strategies are deployed or could be deployed and their efficacy, and how to strengthen One Health surveillance.

Implementing Population-Based Surveys to Study Human Exposure and Transmission Dynamics of Priority Infectious Diseases

 

Despite the growing impact of infectious diseases worldwide and the strengthening of surveillance systems, many gaps remain in our understanding of the mechanisms of transmission and maintenance of causative pathogens in exposed areas. Traditional epidemiological surveillance only provides information on symptomatic cases, leaving asymptomatic or medically unattended infections and the immune landscape at the population level unaccounted for.

 

To complement routine surveillance, the detection of immune responses in serum has been used for many years to determine population infection levels for a wide range of pathogens. However, determining an individual’s infection status and when they become infected is a complex challenge that depends on various factors, including human, virological, and environmental interactions. This information is crucial to assess the risk of transmission, yet studying the immunological status of affected populations remains a challenging task that requires financial, human, and material resources that are rarely available in affected countries.

 

Recent technological advances have transformed sero-epidemiology into a powerful tool that bridges the gap between epidemiology, mathematical modeling, and public health to support risk reduction strategies. The Epidemiology and Public Health Unit is currently developing several population-based serological surveys at different scales to estimate the burden of infection, reconstruct the history of spatiotemporal transmission dynamics, and map infection risks at high resolution. These surveys will inform and guide precision public health strategies to combat priority infectious diseases effectively.
